The work is real and the scope is broad: psychosocial assessments, individualized discharge planning, and cross-continuum coordination across high-volume med-surg, rehab, and LTAC transitions.     Both openings are full-time, permanent positions. The shift runs days — 8:00 AM to 4:30 PM — with rotating weekend coverage required. Staff float across units, and the highest caseload volume falls on the med-surg floors. Candidates should come in expecting a fast pace and a genuinely active role in care transition decisions, not a passive coordination function.

    Compensation is competitive and based on experience.
