    Readmission Reduction: Why Warm Handoffs Beat Checklists Every Time

    The CMS Hospital Readmissions Reduction Program (HRRP) is not subtle. If your 30-day readmission rates exceed expected ratios for heart failure, pneumonia, COPD, or total hip/knee replacement, CMS penalizes you — up to 3 percent of your total Medicare payments. For a large health system, that is millions of dollars annually. And it compounds every year you stay out of compliance.

    The hospitals that consistently beat readmission metrics do not have longer discharge checklists. They have better handoffs. A warm handoff is a structured verbal and clinical transfer of care from the hospital case manager to the receiving post-acute provider — a home health agency, a SNF, or a community partner. The case manager calls the receiving nurse, reviews the clinical summary, confirms medication lists, and verifies the follow-up appointment. That 10-minute phone call prevents the communication gaps that cause readmissions.

    Risk stratification is the foundation. Every admission should be screened using a validated risk tool — the LACE Index, READI, or similar — to identify high-risk patients who need enhanced care transition pathways. Not every patient needs a warm handoff. But every high-risk patient does, and the screening tool tells you who they are.

    Medication reconciliation is non-negotiable. Post-discharge medication errors are one of the leading drivers of readmission. The discharge medication list must be verified against the pre-admission list, reconciled with the attending physician's orders, and confirmed with the patient or caregiver before discharge. Then, within 48 to 72 hours, a follow-up call verifies the patient is taking the right medications at the right doses.
