What You Bring to the Table

Clinical Credentials: Active RN licensure (LCSW/LMSW also considered) with a Bachelor’s degree required; Master’s preferred.

Leadership Tenacity: At least 2 years of case management leadership experience and 3+ years in acute care hospital settings.

Operational Mastery: A deep understanding of discharge planning, regulatory requirements, and the ability to drive clinical performance metrics.

Empathetic Authority: The ability to lead a diverse team of 17+ professionals with a focus on engagement, diversity, and collaborative problem-solving.

Strategic Mindset: A self-starter attitude capable of managing complex cases and departmental budgets with minimal oversight.
