What They Need
Clinical Foundation: A valid, active Registered Nurse (RN) license or Advanced Practice Registered Nurse (APRN) designation with an Associate Degree in Nursing or an RN Diploma.
Acute Care Expertise: At least 2 years of recent, direct acute care inpatient hospital Case Management experience completed within the last calendar year.
Strategic Navigation: Proven mastery in completing psychosocial assessments, coordinating care across multiple inpatient units, and managing Medicare, insurance, and discharge protocols.
Collaborative Drive: A natural ability to interface cleanly with physicians, families, and department directors to facilitate high-quality, patient-centered care transitions.
