What they need
An experienced case management leader with an active RN, LCSW, or LMSW license and a bachelor’s degree (master’s preferred). Candidates must bring at least three years of acute care hospital case management experience and two or more years in a leadership role, with recent hands-on acute care exposure. You’ll oversee daily operations, staffing models, care coordination, discharge planning, compliance, and performance improvement initiatives while leading approximately 15 FTEs. Strong executive presence, communication skills, and the ability to collaborate with clinical, financial, and administrative stakeholders are essential.
