What the employer offers
You’ll join a large, high-acuity acute care environment with the scale and support to make meaningful, system-wide impact. This full-time, weekday leadership role offers administrative hours with no routine weekends, a $10,000 sign-on bonus, relocation support on a case-by-case basis, and eligibility for a strong performance-based incentive plan. You’ll collaborate closely with executive leadership and interdisciplinary stakeholders, oversee hospital-wide case management operations, and have the authority to drive sustainable performance improvement, care coordination excellence, and team development across the entire facility.
What they need
This role requires a strong acute care hospital case management background, including at least three years of recent hospital case management experience and two or more years in a leadership capacity. An active RN, LCSW, or LMSW license is required, along with a bachelor’s degree (master’s preferred). The ideal leader is data-driven, confident managing capacity and throughput, skilled in change management, and comfortable partnering with physicians, finance, nursing, and executive teams. Executive presence, strong communication skills, and a proven track record of operational improvement are essential.
