🧠What We’re Looking For:

Active FL RN license (or compact, with FL licensure within 90 days)

Associate Degree in Nursing required; BSN preferred

3+ years of recent acute care experience in Case Management OR equivalent in Med/Surg, Telemetry, Neuro, ICU, PCU, or ED

Consideration for home health or insurance Case Managers only if 3 years of acute care experience is also met (with 1 year recent)

Ability to coordinate interdisciplinary care plans, manage resource utilization, and lead team-based collaboration

Strong knowledge of utilization review, regulatory compliance, discharge planning, and patient advocacy

You’ll lead efforts that prevent readmissions, ensure proper discharge plans, and align patients with the right resources. Every patient has a story—help write their next chapter.
