Case Manager RN - Day Shift: 8:00 AM – 4:30 PM with Rotating Weekends

Key Responsibilities:

--Assess patients’ clinical conditions and ensure care coordination

--Develop and implement patient discharge plans and treatment pathways

--Work closely with physicians, payors, and the interdisciplinary team to evaluate medical necessity, admission status, and care levels

--Support the transition of patients across the continuum of care

Required Qualifications:

--RN with 3+ years of acute care experience (preferably in ICU or Med-Surg)

--Florida RN license or approved multistate RN license (compact license holders must obtain Florida license within 60 days of relocation)

--BLS-AHA Provider issued card

--BSN preferred, but not required

--Certification in Case Management or Utilization Review is highly preferred
